Hazard, the Belgium forward, has long been linked with Los Blancos and has now penned terms with the Bernabeu outfit after the two clubs agreed a reported £88million fee, which could rise up to £130m due to add-ons.
He leaves Chelsea after seven years, during which time he has won six trophies.

Tottenham have been put on red alert after it emerged Fulham teenager Ryan Sessegnon will not be signing a new deal with the club.
Sessegnon, 19, has one year left on his contract with the Cottagers, who were relegated from the Premier League last season.

Arsenal have confirmed the departure of England forward Danny Welbeck, who leaves the club at the end of his current contract along with Petr Cech and Stephan Lichtsteiner.
Aaron Ramsey, meanwhile, is joining Juventus as a free agent when his deal expires on June 30.
Left-back Cohen Bramall, midfielder Charlie Gilmour, and young Spanish defender Julio Pleguezuelo will also not be at the Emirates next season.
A brief statement on the club website read: “We thank them all for their contributions to the club and we wish them every future success.”
Atletico Madrid are reportedly lining up a summer move for Chelsea full-back Marcos Alonso.
The Spanish defender has fallen out of favour at Stamford Bridge and did not feature in the Europa League final win over Arsenal.
Atletico are in need of defensive cover after selling Lucas Hernandez to Bayern Munich and letting Filipe Luis run down his contract.
Diego Simeone believes Alonso is the perfect man to fill in but it remains to be seen if Chelsea will do business amid their managerial situation and transfer ban.
Swansea have also released a statement on Daniel James’ imminent move to Manchester United.
The Championship club said: “Swansea City can confirm that the club has agreed transfer terms with Manchester United for winger Daniel James.
“The Premier League club has also agreed personal terms with James, 21, following a successful medical at Old Trafford.
“The transfer is expected to be completed when FIFA’s international transfer window opens from June 11.
“The club will make further comment once the transfer has been completed.”
Tottenham have released goalkeeper Michel Vorm following the expiration of his contract.
The 35-year-old joined Spurs from Swansea in July 2014 and made 47 appearances across all competitions for the north Londoners.
Tottenham have also confirmed the departures of development squad players Dylan Duncan, Charlie Freeman, Tom Glover, Connor Ogilvie and Jamie Reynolds.
But youngsters Japhet Tanganga and Jack Roles have signed new contracts and teenage striker Kion Etete has joined from Notts County.
Watford have confirmed that Miguel Britos and Tommie Hoban will both leave Vicarage Road when their contracts expire on July 1.
The club said: “Watford FC thanks Miguel and Tommie for their contributions while at the club and wishes them both well for the future.”
Watford have also revealed that they are in talks over extending the terms of goalkeeper Heurelho Gomes, who is also out of contract at the end of the June.

A statement on Everton's website read: “Boa Morte, who has signed a two-year contract, joins the Blues from Israeli side Maccabi Haifa, where he was also assistant manager, and replaces Joao Pedro Sousa following his appointment as the new head coach of Portuguese side Famalicao.

Manchester United have agreed a deal "in principle" with Swansea to sign Wales winger Daniel James.
A statement on the club’s website read: “Manchester United is delighted to announce that it has agreed, in principle, terms with both Swansea City and Daniel James for his transfer to the club.
“Daniel has successfully completed a medical at the Aon Training Complex.”
James is set to move to Old Trafford in an £18million transfer and become Ole Gunnar Solskjaer’s first summer signing.
Manchester City are reportedly willing to sell Danilo to Juventus if Joao Cancelo moves in opposite direction.
City value their Brazilian defender at £25million while Juve price their Portuguese star at £50 million.
The Sun say a swap deal could be done this summer with the Premier League champions paying extra in cash.
Tottenham have reportedly entered the race for Joachim Andersen and will battle Arsenal for his signature.
The Denmark centre-back is said to have been the subject of two Arsenal bids already this summer, with the Gunners putting in a second €50million offer for Andersen and his Sampdoria team-mate Dennis Praet earlier this week.
Samp are said to be ready to accept the bid, but Tottenham have made their interest in Andersen known, reports in Italy claim.

“Yes [I am flattered by United and Spurs interest], they are really big teams in England and in Europe. To hear those names gives me confidence. I just try to play my game on the pitch and try my best.”
Everton have confirmed that Ashley Williams will leave the club when his current contract expires at the end of the month.
Williams will follow fellow centre-back Phil Jagielka out of the door at Goodison Park.
